New Claim for Disability Insurance Benefits, DE 2501 Form
The Employment Development Department (EDD) implemented a new electronic claim filing system, State Disability Insurance (SDI) Online. The system provides an option to submit claims and forms electronically.
Filing electronically is the easiest and fastest way to submit Disability Insurance and Paid Family Leave claims. All customers can benefit from improved service delivery, which includes electronic confirmations of forms received and access to claim status. To create an account, complete a one-time registration. To register, go to the SDI Online page and select SDI Online Registration. Once the registration is complete, you may log in and complete your claim by visiting the SDI Online Login page.
If you do not file a claim using SDI Online, you must use the updated Claim for Disability Insurance Benefits, DE 2501 Rev. 78 (4-12) form. The new form is designed with Optical Character Recognition (OCR) functionality to work with the SDI Online system and contains additional information that is different from the previous version. To order the new form, visit the Online Forms and Publications page. The form must be completed legibly to facilitate processing and avoid unnecessary delays.
Effective March 1, 2013, Disability Insurance Claims can only be filed electronically using SDI Online, or on the new Claim for Disability Insurance Benefits, DE 2501 Rev. 78 (4-12) form.
