Paid Family Leave Automated Telephone Information System

This information will help you use the Paid Family Leave automated telephone system. Before you dial our toll-free number, please take time to review this information. You will then be prepared to make the selection you need when calling the Paid Family Leave Program.

You will hear a welcome message in English, Spanish, Cantonese, Vietnamese, Armenian, Tagalog, and Punjabi. You will then be asked to:

• Press 1 to hear the recording in English
• Press 2 to hear the recording in Spanish
• Press 3 to hear the recording in Cantonese
• Press 4 to hear the recording in Vietnamese
• Press 5 to hear the recording in Armenian
• Press 6 to hear the recording in Tagalog
• Press 7 to hear the recording in Punjabi

Special instructions or announcements may be played prior to the main menu. A brief description of menu items will then be provided. If you know the selection you want, you can press the number to bypass the main menu recording.

The Main Menu options are:

• Press 1 for information about your benefit payment:
• Status of your New Claim:
  To access confidential new claim information using the automated telephone system, you will be asked to enter your:
• Social Security Number.
• Date of birth.
• Mailing ZIP code.
• Weekly benefit amount or 7-digit home phone number.
  
  You will select your PIN the first time you use the automated telephone system to access confidential benefit payment information.
• Benefit Payment:
  Benefit payment information includes the authorized payment amount, the number of days paid, the beginning and ending date of the payment amount, and the date the payment was authorized. To access confidential benefit payment information using the DI automated telephone system, you will be asked to enter your:
• Social Security Number.
• Personal Identification Number, or PIN.
• Press 2 to request:
• A copy of your Paid Family Leave payment history. We will need your:
• Social Security Number.
• Full name and mailing address.
• Year of your Disability Insurance claim.
• Claim Forms
• Your full name and mailing address.
• The quantity of claim forms you would like.
• New claims can also be filed electronically by you and the physician/practitioner providing care. You will need to provide the physician/practitioner providing care with your electronic receipt number you received when you initially filed your claim online. Log in to SDI Online.
• Press 3 for information about:

• Press 4 for information about:
• Waiting period
• Notice of determination
• Providing care
• Press 5 for information about:
• Elective coverage for the self-employed
• Paid Family Leave contribution rate
