Obtaining Tax Forms and Publications
Most payroll tax forms, guides, information sheets, and other publications can be obtained in any of the following ways (exceptions noted below in the Ordering Instructions for Specific Forms section):
- Most forms are available through your employer account summary page through e-Services for Business.
- Order any quantity through the Online Forms and Publications page.
- Save and print payroll tax forms and publications. Most forms can be filled out using your personal computer. If you need help, see our FAQs Tax Forms and Publications.
- Call our Taxpayer Assistance Center toll-free at 1-888-745-3886.
- Pick them up at your local Employment Tax Office (no more than 25 per visit).
Ordering Instructions for Specific Forms
Payroll Tax Deposit (DE 88) coupon booklet: Your first booklet of personalized coupons is mailed to you approximately six (6) weeks after you register as an employer. New booklets are mailed each year by March. Each booklet has a reorder form on the back cover. You may also call our toll-free number 1-888-745-3886 to order another booklet. Please allow six (6) weeks for delivery. If you need a DE 88 coupon before receiving your booklet, you can get blank Payroll Tax Deposit (88ALL) coupons by any of the methods above.
Quarterly filing forms (i.e., Quarterly Contribution Return and Report of Wages [DE 9], Quarterly Contribution Return and Report of Wages [Continuation] [DE 9C], and Quarterly Report of Wages and Withholdings for Employers of Household Workers [DE 3BHW]) are automatically mailed to registered employers or their designated agents. If you report more employees than can fit on one page, order blank forms by any of the methods above.
The annual filing form Annual Payroll Tax Return for Employer of Household Workers (DE 3HW) is automatically mailed to registered employers or their designated agents. Blank forms may be obtained by any of the methods above.
Claim for Disability Insurance Benefits (DE 2501) is preprinted with the appropriate field office address and may be ordered from a State Disability Insurance Office by phone. You may also obtain the form from your physician/practitioner or employer, send an online message through Ask EDD or visit an SDI Office.
