Using the Automated Telephone Information System
Before you dial our toll-free number 1-888-745-3886, please read the information below to prepare you to make your selection(s) when you call.
Prior to the main menu, special instructions or announcements may play. The system will then provide a brief description of each main menu item. If you know the selection you want, you can press the number and bypass the main menu recording. After you make your selection from the main menu, you may need to make further choices.
The main menu options are:
Press 1 for employer tax service or tax forms.
You will be given seven options for forms and services:
- Tax forms on EDD’s Web site.
- Press 1 for Fax on Demand, forms via your fax machine.
- Press 2 to order forms.
- Press 3 for tax rate or taxable wage limit information.
- Press 4 for Disability Insurance Elective Coverage information.
- Press 5 for DE 6 information, corrections to previously filed forms, or substitute Form W-2 information.
- Press 0 to speak to the next available representative.
Press 2 for Unemployment Insurance or Disability Insurance benefits information.
You will be given three options:
- Press 1 for Unemployment Insurance benefits and Job Service telephone numbers.
- Press 2 for Disability Insurance telephone numbers.
- Press 0 to speak to the next available representative.
Press 3 for telephone numbers of other tax agencies.
- You will be provided with names, descriptions, and telephone numbers for other tax agencies.
Throughout the automated system:
- Press 9 to repeat the main menu choices or to return to the main menu.
- Press * to return to the previous menu.
- Press 0 to speak with the next available representative.
