A return mail address below should only be used when the DE 1101CZ or DE 1101ER is not available, because the address printed on the form may be a specifically designated address used for special claim circumstances (e.g., the claimant's spoken language or out of state residence). Using an incorrect return mail address will delay the receipt of your eligibility information. Mail your DE 1101CZ or DE 1101ER response to the address below based on the last two digits of the employee's Social Security Number, or to the Special Claims Office address if the employee works for one of the State Special Schools or the employee is participating in the Work Sharing Program.
