 Using the Automated Telephone Information System


This information will help you use the Disability Insurance automated telephone system. Before you dial our toll-free 800 number, please take time to review this information. You will then be prepared to make the selection you need when calling the Disability Insurance Program.

For Disability Insurance Claim and General Information Call:

1 - 866 - 658 - 8846 (Spanish)

You will hear a welcome message in English and Spanish. You will then be asked to:
  • Press 1 to hear the recording in English
  • Press 2 to hear the recording in Spanish
Special instructions or announcements may be played prior to the main menu. A brief description of menu items will then be provided. If you know the selection you want, you can press the number to bypass the main menu recording.

The Main Menu options are:
  • Press 1 for information regarding your New Claim or Check:

    • Status of your New Claim:
      To access confidential new claim information using the automated telephone system, you will be asked to enter your:
      • Social Security Number
      • Date of birth
      • Mailing ZIP code

    • Disability Check:
      Disability Insurance check information includes the amount of the check, the number of days paid, the beginning and ending date of the check, and the date the payment was qualified. To access confidential benefit payment information using the DI automated telephone system, you will be asked to enter your:
      • Social Security Number
      • Personal Identification Number, or PIN
      You will select your PIN the first time you use the automated telephone system to access confidential benefit check information.

  • Press 2 to request:

    • A copy of your Disability Insurance payment history we will need your:
      • Social Security Number
      • Full name and mailing address
      • Year of your Disability Insurance claim

    • A form to establish a Disability Insurance claim
      • Your full name and mailing address
      • The quantity of claim forms you would like


  • Press 3 for general information about:

    • Continued claim form or Medical Extension form
    • Waiting Period
    • Notice of Final Payment form
    • Notice of Determination form and your Appeal rights


  • Press 4 for general information about:

    • Eligibility and how to qualify
    • Benefits
    • Pregnancy coverage


  • Press 5 for general information about:

    • Disability Insurance for the self-employed
    • For general information about the employee contribution rate
Mondays and weekdays between the hours of 10:00 a.m. and 3:00 p.m. are our busiest calling times. For faster service you can call outside of these hours.

At any time while using the automated telephone system you may:
  • Press 0 to speak to the next available Disability Insurance Representative
  • Press 9 to return to the Main menu
  • Press the star key (*) to repeat the menu
