Continue or Stop Your Benefits

If you are eligible, you may receive Disability Insurance (DI) benefits until your expected recovery date provided by your physician/practitioner. There are times, however, when you must notify us to continue or stop your DI benefits.

Failure to report your income or a change in work status could result in an overpayment, penalties, and a false statement disqualification.

Continue Your Benefits

If you are on automatic payment, you will receive a Disability Claim Continued Eligibility Questionnaire (DE 2593) after 10 weeks of payment. You must return this form to us to certify that your disability continues. Your benefits will stop if you do not complete and return the DE 2593. To submit the DE 2593 by mail, use the self-addressed envelope provided.

If you have an SDI Online account, you can submit the DE 2593 online. When the form is available to complete, you will receive an email to log in to SDI Online. Select Continued Eligibility Questionnaire in your inbox. You must complete and submit the DE 2593.

If you received the DE 2593 by mail, you must return it to us within 20 days of the mailing date.

  • If you submit this form late, you may lose benefits.
  • Allow 10 business days to process your payment after we receive the DE 2593.

If You’re Not Receiving Automatic Payments

If you are not on automatic payment, you will receive a Claim for Continued Disability Benefits (DE 2500A) every two weeks to certify that your disability continues. Your benefits will stop if you do not complete and return the DE 2500A.

When the form is available to complete, you will receive an email to log in to SDI Online. To submit the DE 2500A online, select Claim for Continued Benefits in your SDI Online inbox. You must complete and submit the DE 2500A.

Note: If you have an SDI Online account and selected electronic as your preferred communication, you will not receive the DE 2500A in the mail.

If you received the DE 2500A by mail, you must return it to us within 20 days of the mailing date.

To submit the 2500A by mail, use the self-addressed envelope provided.

  • If you misplaced the DE 2500A, call 1-800-480-3287.
  • If this form is submitted late, you may lose benefits.
  • Allow 10 business days for us to process your payment after we receive the DE 2500A.

If You Need to Extend Your DI Period

You are eligible to receive benefits until the expected recovery date provided by your physician/practitioner. If you have not fully recovered and need to continue your benefits, you must submit a medical certification. You will receive a Physician/Practitioner’s Supplementary Certificate (DE 2525XX) with your final payment. Have your physician/practitioner complete and submit this form to find out if you are eligible for an extension.

Your physician/practitioner can find your claim in SDI Online.

The DE 2525XX must be returned to us online or by mail within 20 days from the mailing date.

  • If you are eligible to receive continued benefits, allow 10 business days for us to process your payment after we receive the DE 2525XX.
  • If you misplaced the DE 2525XX, request the form using your SDI Online account or by calling 1-800-480-3287.
  • If this form is submitted late, you may lose benefits.

If you have exhausted your available DI benefits but your disability is continuing, you may want to apply for disability benefits through Social Security.

Stop Your Benefits

You must contact us to stop DI benefits if you:

  • Have returned to part-time or full-time work.
  • Have recovered from your disability.
  • Need to report a date of death for a deceased claimant.

Note: To report the death of a claimant, call 1-800-480-3287. You can also notify us in writing using the PO Box address on the payment notice. Provide the following information so we can contact you with additional instructions:

  • Claimant’s full name
  • Claimant’s date of death
  • Your name
  • Your address
  • Your phone number

Benefits are payable through the date of death.

If You’re Receiving Automatic Payments

If you are on automatic payment, you will receive a Notice of Automatic Payment (DE 2587) when your first payment is issued. Sign and return the DE 2587 to us if:

  • You recover or return to work (either part time or full time) before your expected recovery date.
  • You need to report the date of death for a deceased claimant.

After 10 weeks of payment, you will receive a Disability Claim Continued Eligibility Questionnaire (DE 2593). You must complete and return this form to us if you:

  • Recover.
  • Return to work.
  • Return to work or recover but still cannot do your regular work.

If you have an SDI Online account, you can submit the DE 2587 and the DE 2593 online. When the form is available to complete, you will receive an email to log in to SDI Online. Select Notice of Automatic Payment and Continued Eligibility Questionnaire in your inbox. You must complete and submit the DE 2587 and DE 2593.

To submit these forms by mail, use the self-addressed envelope provided.

If You’re Not Receiving Automatic Payments

If you are not on automatic payment, you will receive a Claim for Continued Disability Benefits (DE 2500A) every two weeks. If you have recovered or returned to work, complete Section 1 and provide the date you returned to work or recovered.

When the form is available, you will receive an email to log in to SDI Online. To submit the DE 2500A online, select Claim for Continued Benefits in your SDI Online inbox.
