    This form is used by Employers to certify for the Work Opportunity Tax Credit. This form must be submitted with either one of the following two forms:

    • Individual Characteristics Form (ETA Form 9061) which is filled out by the employer/agent
    • Conditional Certification – Work Opportunity Tax Credit (ETA Form 9062) which is filled out by a state agency or partner.

    Questionnaire to identify individual characteristics of a new employee as one of the target groups to qualify an employer to receive the Federal Work Opportunity Tax Credit (WOTC).

  • Conditional Certification – Work Opportunity Tax CreditETA Form 9062

    This form must be accompanied by the IRS Form 8850.
